Appointments of Enduring Guardian

This document is similar to and compliments the Enduring Power of Attorney in that it allows the person appointed to make decisions on your behalf about your lifestyle when you are not able to do so.

Such functions include where you live, the healthcare that you receive, medical treatment. You can specify as many or as few functions as you would like your Enduring Guardian to have.

Again, because of the permanent nature of the Appointment it is important that the person you appoint is someone who will carry out your wishes.
